The tablet trial evidence
OASIS 4 studied oral semaglutide 25 mg in adults with obesity, or overweight with a weight-related condition. The UK label reports average weight change at 64 weeks of −13.6% with semaglutide and −2.2% with placebo. This analysis accounts for people who stopped or changed treatment.
A second analysis estimated results if everyone stayed on treatment with no extra weight-management therapy. This gave −16.6% and −2.8%. The two analyses answer different questions. Quoting the larger figure alone hides the effect of people missing doses or stopping.
What this means early in treatment
The dose-increase period is one small part of the trial timeline. A few weeks on 1.5 mg cannot be measured against a result taken after more than a year. A dramatic early fall does not mean the same weekly rate will continue either.
Results varied widely between participants. Some lost a larger percentage and others less. Your own review should check whether you can keep up the routine, whether you tolerate the tablets and whether your health is improving in ways which matter.
Work out the change correctly
Percentage weight change is measured against your starting weight. If you start at 100 kg and later weigh 90 kg, you have lost 10%. The same 10 kg loss from a starting weight of 80 kg is 12.5%. Use this arithmetic to understand reports. Do not turn it into a rigid personal target.
A difference in percentage points differs from a relative difference. Comparing 15% with 12% gives a three-percentage-point difference. Calling one result 25% greater sounds much bigger. Comparing separate trials adds more uncertainty, since the people and methods differ.
Follow the trend and the health gain
Single weigh-ins move with hydration, bowel contents, menstrual cycles, clothing and time of day. Weigh in similar conditions and judge the trend across several readings. If weighing upsets you or leads to harmful behaviour, agree another way of monitoring with your clinician.
Health goals include mobility, symptoms, blood pressure, glucose control and eating and activity habits you can keep up. A plateau calls for a review, with no need to panic. The next step need not be a higher dose, a stricter diet or a second weight-loss medicine.
Published averages also depend on who was able and willing to join the study. People with certain illnesses, past treatments or practical barriers can be excluded. Use trial results as a guide to possible benefit, and let your own review cover anything the trial did not represent.
Questions for your appointment
Ask which OASIS 4 analysis is being quoted and why the figure applies to you. Agree when your progress will be reviewed. Discuss what counts as a worthwhile response, given side effects, your health conditions and the demands of the fasting routine.
