The UK tablet steps

The UK label starts at 1.5 mg once daily. After one month you move to 4 mg, then 9 mg, then 25 mg once daily, with at least one month at each level. The maintenance dose is 25 mg. The label allows you to stay on the previous dose if needed.

Take one tablet a day. Never combine lower-strength tablets to make a higher dose. Do not split, crush or chew a tablet to make a smaller dose either. If the next strength is out of stock, your pharmacy and prescriber need to sort out the prescription.

What the steps mean for you

The starting dose helps your body get used to treatment. The first week has no set weight-loss target. Some people notice appetite change early and others later. Neither pattern is a reason to speed up the schedule.

The top dose is no target to reach through significant side effects. Before any increase, report ongoing nausea, vomiting, constipation, poor intake or dizziness. Your prescriber may need to review the plan, especially after a break in treatment or a change to another medicine.

Check before the next increase

Answer four plain questions. How long have you taken the current dose? How many doses did you miss? What symptoms did you have? Are you eating and drinking enough? A calendar month from your first prescription may not equal a month of continuous treatment.

Note any change in other medicines, recent illness or planned procedure. Each can change the decision, even when your weight trend looks good. Check every planned increase against your health at the time. A dose step is no automatic subscription event.

Your dose is a clinical instruction

Your dose does not depend on a friend's weight, how much appetite suppression you want or how many tablets are left in a pack. The dose steps follow the evidence for this medicine and formulation. More medicine does not reliably give a better overall result.

If the pack label, an email and your prescriber's message disagree, ask the dispensing pharmacy to resolve the difference before you take the dose in question. Keep the pack and prescription details to hand. Your written plan should give the strength, daily frequency, next review and what to do if side effects become a problem.

When a new pack arrives, compare it with your written plan before the first tablet. The delivery date need not be the day your increase starts. If you took the previous strength for less time than planned, or had significant symptoms, contact the prescribing service to confirm the next step.

Questions for your appointment

Ask what happens if you do not tolerate 25 mg and whether your next increase should wait until symptoms settle. Check the prescription says tablets, with no mention of FlexTouch, and the daily instructions match the pack you receive.
