No milligram conversion from Mounjaro or Wegovy
Tirzepatide, semaglutide and orforglipron are different molecules with different dosing systems. A bigger number on one pack does not mean a stronger treatment than a smaller number on another. The switch needs clinical judgement. Arithmetic will not give you the right dose.
The licensed Foundayo starting schedule begins at 0.8 mg daily and rises gradually. Having taken another GLP-1 medicine does not entitle you to pick a higher Foundayo dose yourself. If a specialist suggests a different approach for you, they should explain it and record it clearly.
Check new risks before your first tablet
Foundayo's interactions differ from those of your previous treatment. Review your full medicine list, including recent short courses. If you use oral hormonal contraception, follow the current label's 30-day precautions after starting and after each dose increase. These precautions will be new if you are switching from semaglutide.
Confirm the date of your last dose of the old medicine and your first dose of Foundayo. Do not overlap treatments or swap between them to compare effects on appetite. The old medicine stays in your body for a while, so the switch involves more than changing the next date in your diary.
Bring a full treatment history
List each medicine name, form, strength, dates, how you responded and why you stopped. Include side effects, suspected allergic reactions, any history of pancreatitis and any long gap. An order confirmation screenshot is no reliable record of what you took.
If a different provider handles the switch, share your records with your consent and keep your GP up to date. The new clinician needs to know whether your previous treatment failed, caused side effects you were unable to tolerate, was unavailable or was inconvenient. Each reason shapes what a good switch looks like.
Agree the plan after the change
The plan should give your first dose, the dates for dose increases or reviews, advice on symptoms and who to contact if you cannot tolerate Foundayo. If you have diabetes, the plan should also cover glucose monitoring and your other medicines.
Do not judge the switch on the first day's appetite or one weigh-in. Do not sit on severe symptoms while you wait for treatment to settle either. Follow-up should check progress towards your goal and whether the new routine works for you. A dispensed prescription does not prove success.
Store any leftover supply of your old treatment separately until your pharmacist advises on disposal or another plan. Two similar packs in use at once raise the risk of an accidental overlap. Update reminders and repeat orders to the new routine so they stop prompting the old one.
Questions to take to your appointment
Ask for the full switch schedule in writing and a fresh interaction check. Confirm contraception precautions where relevant, your starting tablet strength and the plan if you stopped your previous medicine because of a significant side effect.
