What the researchers compared

OASIS 4 was a randomised, placebo-controlled phase 3 study. Participants took oral semaglutide or placebo, and the main body-weight comparison came at week 64. Keep the population and schedule attached to any figure you quote. The daily tablet is a different product from the weekly injection in tablet form. Its exposure depends on a purpose-built formulation and a set way of taking it.

The main weight result

Estimated mean weight change at week 64 was −13.6% with oral semaglutide and −2.2% with placebo, a difference of −11.4 percentage points. These figures answer the treatment-policy, or treatment-regimen, question in the main analysis. Analyses that assume everyone took treatment as intended give different numbers. A good summary names its analysis instead of picking the largest percentage.

The daily routine is part of the treatment

You must take oral semaglutide by its fasting, water and waiting instructions. Those rules affect how much medicine you absorb and how practical the tablet is for you. So discuss the trial result alongside whether you can follow the routine every day. If the routine is hard, ask for a review. Never take extra tablets to make up for a missed or mistimed dose.

What OASIS 4 did not compare

OASIS 4 did not compare the tablet head to head with Wegovy injection, tirzepatide or orforglipron. Similar percentages from separate studies do not prove equal or better effect. Differences in participants, trial length, how many people stopped and statistical methods all change results. A well-run indirect comparison can explore some questions, but it rests on assumptions the authors should spell out.

Approval, funding and choice

The MHRA approved Wegovy tablets in June 2026, giving them a UK authorisation. NHS commissioning and individual prescribing follow their own processes. OASIS 4 does not mean every adult with a weight concern should get a tablet. A clinical assessment still covers eligibility, medical history, other medicines, side-effect risks and whether you can keep up the treatment over time.

What you can take from OASIS 4

The trial supports oral semaglutide as an effective weight-management option in the studied setting. OASIS 4 promises no particular result and does not remove the need for follow-up. Ask your prescriber how they will judge both benefit and tolerability, and what happens if daily dosing or fasting becomes difficult. To compare with another product, ask for evidence on the actual product and dose under discussion.

The 25 mg dose is the target, not the start

"Oral semaglutide 25 mg" names the target regimen in OASIS 4. You do not begin on that amount. Trials include a planned introduction and monitoring process, which short summaries often hide. For a real prescription, follow the current authorised starting instructions, never a study's maintenance-dose label. Ask how the first weeks will run, which symptoms mean you should get in touch and how missed doses affect the plan. A tablet still needs dose escalation.
