Why the 0.5 mg stage exists

The dose is higher than 0.25 mg but still part of the gradual build-up. The standard maintenance dose comes later. Each stage helps your clinician judge whether you can tolerate treatment over time. You face no weight-loss deadline at 0.5 mg.

An easy first month does not guarantee a symptom-free increase. Doubling the dose does not double side effects or weight loss either. Your body's response does not scale with the number on the box.

Check your new pen

Confirm the pharmacy has supplied a 0.5 mg pen and the prescription matches your agreed plan. Different strengths can look alike at a glance. Keep used and unused boxes in order so you are not choosing between several uncertain pens on injection day.

A UK FlexTouch pen delivers its labelled weekly dose when you follow the instructions for that product. Do not make your own click chart, draw out leftover liquid or assume another brand's pen works the same way. If the dose counter will not reach your prescribed amount, the leaflet tells you when to start a new pen.

Watch how you function as well as your appetite

A good increase leaves you with a manageable eating pattern and a normal day. Ask yourself whether you can eat enough, drink normally, concentrate at work and do ordinary activities. Feeling unusually weak because you eat so little needs attention, even if your weight is falling.

Some people find hunger comes back towards the end of the week. This is no reason to inject early. Note the pattern and raise it at your review, especially if it leads to erratic eating or worry about the next dose.

Act early on digestive changes

Mild nausea or bowel changes may appear around a dose rise. Smaller meals and enough fluids are sensible first steps. Get advice for repeated vomiting, significant diarrhoea or constipation which will not ease. Ask a pharmacist before adding a medicine for symptoms, especially if you take other regular prescriptions.

Do not step up again on the belief you must put up with severe symptoms. Your prescriber may slow the increases or change approach. Severe abdominal pain which will not settle, or being unable to keep fluids down, needs urgent assessment. A routine repeat-prescription message is the wrong place for it.

Planning the next month

The usual next stage is 1 mg weekly after four weeks on 0.5 mg. Tell the service about missed injections and any break in supply, because weeks on the calendar may not match doses taken. Keep a simple record of your four injection dates.

If your weight has barely changed, review the whole picture before calling treatment a failure at a build-up dose. If you are losing weight fast while eating poorly, review that too. Both situations need an individual decision. A subscription renewal should never trigger an automatic increase.

Your own medicine leaflet and prescriber come first. A source from another country does not mean the same rule applies in the UK.
