The study design
The phase 3 study tested daily orforglipron at trial doses of 6 mg, 12 mg or 36 mg against placebo for 72 weeks in adults without diabetes. The publication names these doses. Trial formulations and commercial tablets can differ. Never copy the paper's milligram figures into a current dosing schedule without checking the authorised product information.
The published main result
Mean body-weight change was −7.5% with 6 mg, −8.4% with 12 mg and −11.2% with 36 mg, against −2.1% with placebo. Every active group lost more weight on average than the placebo group. The numbers describe a study population and an analysis. They guarantee no individual result. Tolerability and the number of people who stopped also shape what the treatment achieves in practice.
Why trial and pack strengths differ
A development programme can change the formulation used to deliver a molecule. If absorption changes, the tablet may need a different amount to reach the same level in the body. So an old trial dose need not match the number on a later commercial pack. Your prescription and the current leaflet set your dose. Never combine smaller tablets to recreate a research dose.
Convenience and effect are separate questions
Orforglipron does not carry the same food and water rules as oral semaglutide, so some people may find daily use easier. ATTAIN-1 does not show that everyone will stick with it better or prefer it. An easy routine matters because people keep it up. Weigh that preference alongside clinical suitability, side effects and your treatment goals.
Do not rank separate trials
ATTAIN-1 and OASIS 4 differed in design and follow-up. Setting their headline weight percentages side by side without adjustment gives no randomised comparison of Foundayo and Wegovy tablets. The same problem applies when ranking orforglipron against an injectable medicine studied elsewhere. If someone cites an indirect comparison, ask which populations were matched and which assumptions remain. Do not treat the figure as a direct trial.
What authorisation means in the UK
The MHRA approved Foundayo for weight management and type 2 diabetes in August 2026. The diabetes indication rests on its own evidence, so do not infer it from this obesity study. Authorisation does not settle NHS funding or whether Foundayo suits you. Your prescriber should explain the licensed schedule, when reviews happen and what to do if treatment stops or causes problems.
Find the analysis behind "up to"
A phrase such as "up to" may refer to the highest-dose group, a particular estimand or a subgroup. The figure may not describe the typical result for everyone offered treatment. Read the table or methods to see what was selected. In ATTAIN-1, each dose group and the placebo group show part of the picture. An honest summary does not pair the best efficacy figure from one analysis with the best tolerability figure from another without saying the two answer different questions.

Your own medicine leaflet and prescriber come first. A source from another country does not mean the same rule applies in the UK.
